Dementia Friendly Facilitators will work with individuals and their families to develop person-centred plans. These plans will guide personalized programming and activities that will be facilitated in the individual’s home and community. By engaging in people’s communities, the program also aims to increase overall awareness of dementia, thus creating dementia friendly communities.

The video was created to raise awareness of those impacted by dementia in rural Saskatchewan. The following stories highlight personal experiences of how the Dementia Friendly Life Enrichment Program has improved the quality of life for persons living with dementia and their care partners.
